I think code is 3/4 inch step up? Not sure. I had an inspection fail for a rise about 1-1/2 inches and we had to fab a metal ramp.

If you start with something like this yo might add another pieces to the bottom.

Or modify it like this.

 

Here is a 3/4” reducer from Lumber Liquidators. Maybe use this and live with the last 1/8”. Or shim the back with something.

You really want something more gradual than quarter round—you'll trip over it and beat it up over time.

 

Based on the suggestion of a Lowe's rep, I put a seam binder under a reducer. There is still a small bump at the end, but I think the trip hazard is reduced now.

The wood stain I got didn't match the floor, but this is good enough for me.
