Recently bought a DeWalt DCD995M2 kit. A drill bit would comes loose which at first I though was just me but come to find out that many people but "not" all have this problem with DeWalt brush less drills.
In another forum, a link was posted to this video showing the problem:
Some suggested that it's the quick action spindle brake causing the problem but it also happens in reverse just takes a few more start/stops to drop the bit. It doesn't happen in any other speed but high on this drill.
I have an email into DeWalt right now but no answer yet but I've read that they deny the issue. Further reading shows that this is a problem with not only DeWalt but Ryobi, Makita and Craftsman some of which aren't even brush less models.
Before discovering that it was a operator error I purchased the same chuck and put it on an old DeWalt 14.4V drill which has had a key chuck installed since new. Thinking since not everyone has this problem I'd swap chucks but I'll be darn if I can get that chuck off! They had Loctite on the locking screw but I don't know if they use some on the chuck threads also.
