Best way to add ventilation to an enclosed patio/sunroom

We have a 12x12 sunroom/enclosed patio on the back of our house. Ever since we moved in 6 years ago, there's been a musty smell in there. This spring, I decided to just gut the whole thing and start fresh. Its currently ripped down to the studs, and everything has been treated for mold/mildew. I'm about to put things back together, but I'm wondering if I should add some ventilation to the room since there is none. My question is, what's the best way to add that?

The room is 12x12, and has a sloped roof that rises to meet the rest of the house. There is no vent in the roof, and no soffit vents on the roof overhang. The roof is shingled and the siding is all vinyl.

Any ideas?

Do you heat it in winter?
Heating in winter fills the room with warm air that holds more moisture and becomes an issue as it leaks into the small attic space.

What is below the floor I see? Is there an open space with dirt below or is that a slab?

What do you plan for the walls and ceiling?

Please post some pictures of how the outside was done.
Just a few of the dozens of things I've seen done wrong by DIY'S and even so called pro's when they try and enclose an old slab patio, no overhang on the roof, no gutters, slab is too close to grade, walls were built leaving the slab sticking out, roof was not built with enough slope because it was not built up on the house roof.
