What kind of roof protrusion is this?

I'm making a list of materials I will need including flashing. I have a bunch of standard mushroom vents and a couple 3" pipes, but also one of these trapezoid looking vents. I'm not sure what to call it.

Can you help me ID it and track down the correct flashing?

Search internet for Goose Neck Vents or Roof Caps.
The flashing is built into the vent, no other flashing needed.

Awesome. I assume it's preferable to leave it be unless it appears damaged?
You don't need flashing for those. Just ice/water around the protrusion through the roof and install the vent.

Good link on proper install of penetrations.

Exhaust fan hood, and I would recommend changing them. It looks like vinyl, which is fine but not something I would reuse. They aren't expensive, vinyl ones are under $20 in my market. Cheap insurance. +1 on Frank's install guide, I'm glad his website is still around. It's a great resource
