I’m replacing a bathroom. It was linoleum, it’s going to be wood.

I got super lucky when I pulled up the linoleum. I saw tile, which freaked me out, but the tile is on backer board and the backer board is nailed down, no thinset.

The floor in the bedroom (through the bathroom door) is carpet and pad over 3/8 plywood subfloor.

This second story floor is 2x6 t&g over 8x6 rough sawn floor joists, 48" on center. (This floor is the ceiling for the lower floor; this is post and beam construction.)

Should I add a subfloor? Or just put my flooring over the existing floor? (I will run it perpendicular.)
If I need subfloor can I use 1/4" plywood? Any particular nail pattern?
If I don’t need sub floor (or if I do) do I put anything between the floor layers? Paper?

You say wood, but that can mean different things. Nail down hardwood? Engineered? Floating?

I would say the only reason to add plywood would be to raise the elevation of the new floor up to match the existing. Or maybe to prevent hardwood nails from penetrating the ceiling below, if there is a v groove for example. The other reason would be if the existing subfloor isn't smooth enough for whatever you are putting on it, likely not an issue with wood.

Use 15 lb roofing felt as an underlayment over the planks. What sort of unsupported span do you have under there? Obviously it was substantial enough for the tile, it should be fine for the hardwood. Good luck with wood in a bathroom. Not the best choice.
