I know, its the flapper getting old and hard, right? But, no, it isn't. i must have fixed a couple dozen commodes over the years but this one has me baffled. I replaced the flapper and it works... sometimes, just like before I replaced the flapper.

Then starts leaking by again so the the fill valve runs every couple of minutes to top it off. I have watched this over and over and there is no issue with the chain or handle hanging up and the flapper looks like is seats perfectly every time. Sometimes it leaks and refills constantly, sometimes it works correctly and doesn't leak by.

I have also felt around the circumference of the flush valve seat where the flapper lands and it seems fine. No cracks or rough spots.

Anyone ever run into something like this or have some ideas? It is driving me nuts.

If you are sure the flapper is 100% operational (did you do a dye test?), then look at the tank-to-bowl gasket, and to do that, you have to remove the tank. Just have a new (sponge style) ready with you and make sure it's the right size before you start.
BTW, did you notice any leaks outside the bowl?

Make sure that the refill tube (that little black tube from the fill valve to the overflow pipe) is attached to the top of the overflow pipe and not stuck down into the overflow pipe.

No external leaks. The tube is properly clipped into the top of the overflow tube. It really has to be leaking by the flapper somehow not aligning properly when it closes... but I'll be damned if I can see a difference between when it is leaking by or not. The question is why. I made one more small change, adjusting the flush arm to be more centered over the flapper so it will hopefully fall down absolutely straight. So far I have run 3 test flushes and we are good. But it had been good for a week after replacing the flapper last Saturday.
