if you only need a couple of inches you can use an offset flange with very little concrete demo. if it needs to go farther, then you will have to cut a trench in the slab and go from there
I had a similar problem and learned that not all toilets are the same width. Fortunately I was replacing the toilet and was not concerned with keeping the old one. You might be able to save 1 1/2" with a smaller (less wide tank) toilet.
