I am going to run NM sheathe 12/2 w/ ground from panel box ( out side mounted ) into my attic. The first 5 feet of the NM cable will be in schedule 80 pvc from box into attic. What ID of schedule 80 PVC do I need for the proper fill rate?

You can't run NM cable outside a building since that's considered a wet area regardless of the type of conduit you use.

You will need to run #12 THWN or THHN/THWN wire... a white, black and green will do. Any 1/2" conduit is fine.

You can junction that in the attic and run NM from there on if you like... just provide a j-box with proper fittings and cover lid.

Wet area rated cable can be sleeved in conduit or possibly left exposed
depending on requirements for mechanical protection in your situation
and local codes.
Then there's armoured and wet rated cables...........

Because of the low ranch style roof and 16"-18" of blown insulation, I don't want to deal with conduit on an un-floored attic. I will use 12/2 UF-B in conduit from box to attic.

Schedule 40 or 80 ? and what size I.D.?

Above the panel is not subject to damage so Sch 40 is fine. 1/2" is fine. Come out of side or bottom of panel, not the top.

This is the proper way to run your circuit.

There is more than 1 proper. UF sleeved in conduit is perfectly compliant and easier for the poster. Many poster state their opinion as code.
