Simple handrail questions

If I want to put a cheap pine handrail on a wall, but want it to sit about 5 or 6 inches away from the wall, are the larger, extended brackets out there? Or should I put wood on the wall and attach the brackets to the wood in order to make it extend farther away from the wall.

I hope that makes sense.

General spacing is 3 1/2" from the outside of the handrail. Longer standards may be weaker. Why is it that you need that spacing?
