Simple handrail questions

If I want to put a cheap pine handrail on a wall, but want it to sit about 5 or 6 inches away from the wall, are the larger, extended brackets out there? Or should I put wood on the wall and attach the brackets to the wood in order to make it extend farther away from the wall.

I hope that makes sense.

I did that very thing on our rail, but only the lower end, because of necessity of where the column post needed to be positioned in the basement. The basement ceiling is concrete and to trim it out the post had to be there. gave the extension the same router profile as the other wood work.
