Is there a standard size bored hole I should use when running 12/3 (and some 12/2) through a series of 2x4 studs? Or is it just a judgement call?

If I put the hole dead center (more than 1.25" from each side) I do not need nail plates, correct?

Do I need to secure the cable to the wall with a staple before it enters a receptacle box? Or does the bored hole that will be just above the box count?

I use a 3/4" bit.
Nail plates not needed if center of stud.
And cable going into a box needs to be stapled.
