Removing tape from woodwork :(

Why do I have so much trouble removing painters tape from woodwork?There are always areas that the paint comes off the wall! I may as well forget the tape and cut everything in, I have to go back and touch it all up anyway.

Wrong tape,
improper prep for paint
not enough cure time for paint

any or all of the above
