How do I build up tile for my new kitchen floor

The old kitchen vinyl floor was torn out as well as the 3/4in subfloor, still leaving the 3/4in plywood on the joists. This plywood was very loose(loose nails) so all edges were screwed into the joists. we want to tile the floor now to match the 3/4in hardwood floors which surround the kitchen in the adjoining rooms. What should we use as an underlayment,(plywood or cement board) and in what thickness if we want to install a 1/4 in tile? Also would it be good idea to install DITRA? If so then what thickness to build up to height needed and what underlayment is recommended then? DITRA states it is 1/8in thick. Thank you
