It's not one piece. It looks like a manifold with 4 regulators hooked up to it.

Definitely some type of manifold with side input. I've seen something like that in the big box stores in the sprinkler section.

Take it to a plumbing supply store and let them try to match it up.

Another suggestion, you could very easily make one that would work if you are crafty enough. The box stores have all the parts. Just a suggestion.

After I posted my response I looked at it again and thought a gas control valve for a four burner stove or BBQ.

Steam heat manifold? You may have trouble getting an exact replacement but there are several hits to check out.
