Not entirely sure what this is called. These are the lines from the condenser going to the inside of the house.

I was told this was not done very well. Not sure how this should look and how I should fix it.

The foam is brittle and appears to be rotting so I probably need to replace it.

Looks to me like you need the services of a competent brick mason. Once you get the hole size down to something more sensible and aesthetically acceptable, you can seal the penetration with a small bead of RTV silicone which will endure the elements much better than any foam you can get.

If the insulation on the suction is foam type and crumbling to the point that it's bothering you, replace it with a piece of Armaflex pre slit or Frost King foil backed pipe wrap.
