According to the Architectural Graphic Standards The height is 42" from the shower floor. and the head should be between 65" and 78", I usually set this height at 74"

Valve should be at what height?

diverter should be how far above valve?
Nothing cast in stone except ADA req. and tub spout which needs to be- I believe- 4" above the tub rim to meet code. Beyond that it's a matter of preference. Tub/shower valve is apprx 12" above spout. For shower only, Joe's numbers are good. The last thing you want is to bend your knees in the shower :yes:

Sorry guys, when I say diverter, this is a shower only and what I am calling the diverter is Kohler k728 and it changes from head to handheld then both.

I'm 5'8" you may be 6'4". I'd put the head where it works for you.
Just stand there with it in your hand and try it.

Minimum requirement is that the trim plates don't hit. It's that simple
