I am trying to diagnose a problem where the hot water from the shower turns lukewarm after 5+ minutes. This occurs throughout the house as well. The hot water temperature drops. Here is what I have performed and/or observed.

-Replaced the Delta Monitor trim shower system. I thought it was the cartridge that mixes the hot/water. This does not fix the problem.
-Flushed the water from the bottom of the water heater to remove sediments
-The cold inlet tube that brings cold water into the water heater is not cold. I have another water heater and the cold inlet tube is cold.

Does anyone have any advice or suggestions?

You have three (3) Hot water Heaters???????

Sounds like the dip tube is shot

That would be your problem, bad lower element / bad or no Dip tube
