I am trying to identify the pipes for my basement bathroom rough in. Any ideas? The far pipe with the 12"x12" knockout is 2" and i am guessing it is for a shower. The other narrow pipe is a 1.5" pvc pipe. Any help would be appreciated, my last resort will be jackhammering.

Thanks for the input. What do you mean by 3" clean out and toilet / lav waste? I am having problems visualizing what is going on under the slab could you sketch it?

The end of the building drain needs a clean out by code, In your case, it also needs to be 3" So you put a clean out tee on that riser at what ever height you want. Then reduce the top of the tee to 2" and now it is the toilet vent.
If you want a lavatory sink to the left of the 3", you can add a tee to arm down the wall to your lav location. The lav will drain into the 3" clean out/toilet vent. This is known as wet venting. Hope this helps :)

See my sketch below. Is this what you mean by wet venting? Thanks for all the info it looks like i wont need to jackhammer any concrete!

That looks right. you could lower the clean out so its near the floor- bush it down to 2" then add a 2x2x1.5" lav tee on top- only the clean out needs to be 3". That would keep you from buying a 3x1.5 tee (spendy). Put the lav tee at 19" above the floor (center of the branch outlet) That will work for most lav heights

The 4 inch is for the toilet.
The 3 inch is a vent stack.
The 1.5 inch is for a vanity (although, it could be a re-vent).
The 2 inch is for a shower (You'll have to trap that - under the concrete
floor)
If the 1 1/2 inch is too close to the shower (for a vanity) - you could move
the vanity toward the 3" pipe - "T" off the 1 1/2 inch pipe, run it into the
3 inch pipe, as a drain for the vanity.
Off the top of the "T" on the 1 1/2 inch pipe - run it into the 3 inch pipe as
a re-vent (above the "flood-rim" of the vanity).
As "TheEplumber" says - "clean-out" on the 3 inch pipe.
