I'm running an underground cable to a 100A Main lug panel from the disconnect/meter at pole and need to know if I need to run an extra wire for ground if I'm using 2-2-4 URD. Do I even need to run a ground? I don't know what the requirements are. And if I do where does it hook up to in the panel?

You buy a ground bar and install it in the panel. You may also need a ground bar lug adapter.

Why are you using a main lug panel instead of a main breaker panel ? It sounds like this is a run to a detached structure, so a disconnect would be required there.

The service is just for a small shelter at our lake property so I didn't think it needed a main breaker. I'm only using 4 spaces in the panel.

A ground (4 wire feeder) is required, why don't you run quadplex URD? If the main lug panel is service rated, 6 or less breaker throws qualifies as a disconnect.

Where is this located??

That's not like any disconnect/panel I have seen in the US.

This is my setup. Disregard the smaller wires, it was a temporary outlet. Where will my 2-2-2-4 Aluminum cable conductors connect in this disconnect?
The neutral and ground will connect in the exact same spots as your temp lines. The two hots to each side of the live bars, again, like the one temp line.

In this main disconnect the neutral and ground are bonded and connect to the same place, in a sub-panel (non-service panel) they must remain isolated with the ground bonded to the enclosure but the neutrals isolated.
