Our patio is flagstone with mortar between stones. The mortar and many of the stones are loose.
I thought I could remove some of the mortar and stones and plant groundcover in the place of the removed stones and mortar. I removed some stones and mortar; there was about 1/4 inch flagstone 2 inches of concrete, then another older layer of concrete. I do not know how deep the lower layer of concrete is, or what is beyond it.
Could I plant on the large areas where I removed the stones?
What ground cover should I use, and what substrate should I fill the holes with to plant the groundcover in?

I agree - remove all of the flagstone and concrete and re-pour the patio - gives you a nice clean base to do with as you (actually the Mrs.) please - stamped, stone again, lots of nice patio ideas out there to choose from.

Maybe some clarification or some pictures. The flagstone was 1/4" depth (?), and was sitting on a 2" mortar bed over a slab of concrete? And you flushed the mortar joints down to the mortar bed? You want to fill just those 1/4" (?) deep holes of some removed flagstones, or the whole patio, with groundcover or a "substrate" then a groundcover on top of that?

Either way, you should have a min 3"-4" of soil for turf grass or low height perennials, that is well drained so when the water leaves air fills the voids to allow nutrients to the roots.
