How do you apply the self leveling cement grout into a crack on the patio. Do you just carefully fill the crack or fill it and spread it out over the good cement with a putty knife??

Leave it alone. It will just make it worse trying to fill it with concrete. If you must fill it use something soft like caulking.

Or use an epoxy product especially made for concrete restoration. You can get everything from crack injectors to filler of different kind. Here is one source.
