I have a big spot on my bathroom wall that has been mudded and painted. The spot looks like it needs to be sanded. What is the easiest way to fix this considering it's already painted?

Can you mud over it and spread it out.
The mud will stick to the paint.

if it was just done then the mud will stick to the paint. when the paint is old and theres a bit of texture to it the fresh mud you put on will bubble becuase it cant adhere to the paint. before mudding the spot prime the area you think where all your mud will go so you know the mud will stick.

Sand as best you can (paint doesn't sand well), skim another coat (or three) to smooth it out, sand again and paint. When you skim, a couple of thin coats are better than "piling" the mud on to try to cover with one coat.....
