This is a unique fireplace to say the least! At first I wanted to remove all of the brick and create a more updated fireplace flush with the wall. My contractor talked me out of that so now the plan is the keep the fireplace intact with a whitewashed coating and some kind of crown moulding around the top.

First, would you add a crown moulding to create a cleaner look of a traditional mantle piece? If yes, do you have any suggestions or photos of a 'curvy' fireplace with an attached mantle?

Second, would you leave the top edge as is without adding the moulding?

You could always cut a piece of crown molding and hold/set it in place to see if you think you'd like it. Neal is right, that is an unusual/different fireplace, I've never seen one like and I've painted 100's of homes with fireplaces.
