When entering a breaker panel, do you need to use cable clamps at the top to secure the wire, or is it up to code to just knock out the temp plug and run the wire in.

You need the clamp.

And it needs to be the proper connector for the size and type of cable.

The reason I asked, was because I see someone didn't use a clamp on one of the wires, running down to a 15 amp breaker on my panel. I'm guessing it was an addition at some point done by the previous homeowner. I'll have the electrician fix it up when he's out here.
