I have an 11-year-old aluminum patio screen door that keeps getting stuck on a broken section of the bottom plastic sliding guide.

This is what the plastic guide looks like when removed from the patio screen bed.

That is the broken section of the guide.
I'm trying to figure out how to repair the plastic guide, make a substitute, or possibly get a replacement part from the manufacturer. The problem with the latter is that I am not sure of the manufacturer and model number as the labels have been rubbed off:

I would appreciate any suggestions for fixing the screen.
