I know this is a DIY forum, but does anyone have a rough idea what a plumber would charge to run the water / drainage for a toilet / tub / sink in an attic bathroom? Everything would probably have to go up two stories.

What diameter pipe would need to be ran for the drain?

Too many variables to estimate especially geography (Beverly Hills is going to be higher than a lower income area). Get three estimates from licensed, bonded, etc... plumbers.
I'd assume it will be > $2k but that is a WAG.

This might be my only option - I just hate to have people come out and give quotes when I'm not really ready to have the work done. Not even sure it's possible to add a bathroom in one of the places I want (without running pipes through the rooms below it)

I agree; it is a waste of your and more importantly their time. When you've read some books and got estimated cash, start calling. After reading, maybe you'll want to do it yourself?
