My in-laws have an island sink. It drains slowly, I believe an AAV under the sink will help greatly. The issue is, the vertical pipe from the basement comes up and then a 90 turn to horizontal run of about 12" then another 90 turn and a horizontal run for 8" and then the trap.

The only easy place to put a T to an AAV would be after the second ninety, right before the trap---would this be OK?

There are no clogs.

The T would be vertical not horizontal. Is it OK if it is vertical after a horizontal run, as long as it is before the trap?

Best place for it is at the vertical to horizontal 90. If you can't replace that 90 with a tee- AAV on top then any place after that and before the trap will have to do
