Off set flange help

I everyone I need some help with this plumbing in my basement washroom.
I'm trying to install an offset flange toilet. After I broke the concrete i found some problems. If you see the pictures the rough-pipe is 4" and my flange is also a 4", so I do not which is my mistake.

The offset flanges comes in different types of measures?.

How can I solve this?

Thanks!!

Your toilet needs to be 15 inch from the side wall and 12 inch from the back wall. You not moving it that far with any offset flange.
