Converting 4" cleanout to vanity


Hi,
I am remodelling a basement bathroom and would like to reconfigure the location of the sink. When removing all the gypse, I found a 4" cleanout that was covered, which lines up perfectly to where I want to install the new vanity. The cleanout is for a 4" drain that is the main drain of the house for 2 other bathrooms going up 3 floors.
Originally, my ideas was to cut into the 4" drain and use a Y fitting that has a reducer on it that accepts vanity drains. But given it is a main drain going up20 feet, I don't see how I can fit in the Y. The next idea was to convert/reduce that 4" cleanout into a 1 1/2 drain. I don't want to lose the cleanout though.

Can I somehow use the 4" cleanout as a vanity drain, while still maintaining a cleanout? I was hoping there was a fitting that connects to the 4" cleanout and provides another 4" cleanout but also with a vanity drain.

I included a picture to better explain since I am new to plumbing. The vanity would be located on the right of the main drain, exactly where the cleanout is pointing. Any help would be greatly appreciated

The cleanout is there for a purpose and it's not to be modified into a drain for a sink.
It should also be accessible in case a blockage occurs.
