Fishtape refill

OK, now I feel stupid! Can anyone tell me how to refill a fishtape? I thread it properly but the looped end on the inside keeps popping out when I reel the tape in!

all of them I can remember fixing allowed for the placement of the inner loop to be held in place by one of the cover screws.

it is captive at that point and cannot come loose.

or more simply put:

one of the screws goes through the loop on the end of the fishtape.
