big spark from hot feeder when install a water heater

i installed more then 10 water heater with no problem before until this one, i think everything was normal, but at the moment i turned the breaker on, then a big spark happened just like an explosion (big sound and spark). The hot feeder was beated up at 1 inche away from the connection between the heater's black wire and the black feeder. But the breaker did not trip. I dont know what caused that?

You probably had two wires accidentally touching. INsulation may have been worn away allowing the bare wire to touch something else. Also it is easy for stranded ends of a wire to poke over to the next screw or to other metal parts. Do you have a picture of exactly where the spark happened?
