Attic Fan E62815 EPC

E62815 EPC

I have the detailed Fan installed at my home and it has stopped working, (was not serviced ever and went through it's life cicle)

I would like to buy a new one, but exactly the same dimentions so that I do not change all the installation that there is set up for it, the round hole where it is mounted is 15 inches in diameter and the fan is a four blade, just under 14 inch diameter, the motor is 5 inches in diameter and about 3 inches thick, it's a 115 volt 60 HZ 4 amps unit.
It seems to come attached to the adjustable temperature thermostat but what has failed is the electric motor.
