Tree tipped due to storm - how to straighten, save?

During the late winter storms, my tree got almost uprooted and is bent over (see picture), also in the pics you can see the roots came up and now some of the leaves (in July) are yellowing.
Does this mean that the tree is dead?
Is there a way to straighten the tree?

Sometimes you can hook a chain/rope to them [protect the bark!] and pull them back straight. This would have been best done as soon as the ground thawed back in the spring. IMO you have nothing to loose - straighten it out, support it and see how it does.

I tried to move it back now but it's too heavy (or did I wait too long and there's another time to do it?)

It's easier to do when the ground is wet/soft. Mechanical help is often needed.

I would go ahead and take it out. Losing time on having a healthy tree planted in its place that could be getting tall and nice.

Even if you stand it up and it survives it will take years for it to establish new roots to be steady on its own. I suspect it will always need to be held in place with cables.
