I would take up some the flooring and sub-floor a small piece at a time. You might be surprised it may be a smaller job than you expect. Just don't cut through a floor joist.

perhaps possible to pump crawl space to a specified height using cheaper cement based "grout", then rip up floors and pour concrete.

doable yes. a job+ indeed, and expensive.

you need some sub-floor repairs AND want conc flooring ? simple - replace nec sub-flooring, have durock put down, & then place thinlift [ eg, mapei ] liteweight conc on top you can stain it, dye it, &/or polish it
