What kind of vent is this?

What kind of vent is this and where can I find a replacement? It is in bad condition and needs to be replaced. Thanks!

If you mean the vent on top of the chimney, it could be venting any number of things such as your furnace or hot water tank. Interesting that it appears to have FOUR outlets. I'm sure a roofer will be along to give you the proper name. I'm sure you could find an ample replacement at your local heating and AC distributor or even at your local Big Box hardware store. Just take a pic with you to show them what you need.

The chimney portion is the part that needs replaced. It has deteriorated roofing cement on all sides.

Just looks like a 4" gas vent cap. Get one most anywhere, even big box stores.
Best to remove it and take it with you. It should only be attached by two small stitch screws.

And, yes, looks like your chimney could use some attn too.

If all the chimney is doing is venting your furnace you can also remove it below the roofline and flash the vent into the roof. Would be cheaper.

Looks like a fake chimney that someone ran a HWH or furnace flue through. I would remove the chimney, install a tall cone flashing and rain collar, and shingle it in. The flanges should be enveloped with Ice Dams Protection Membrane. I'm not sure if this is on the front or back slope of your home. I would re-route it to the rear slope. Definitely get rid of that hideous looking fake chimney, or whatever it is.

It looks like its venting the HWH. If I remove the chimney can I just run a straight pipe?

IT looks to be a masterflow vent cap.
