Rain water coming in through bathroom exhaust fans

Whenever we get a hard rain, water gets into the bathroom exhaust fans and drips down into the room... Any thoughts on hoe to fix this issue?

Post a picture(s) of where the vents go through the roof.
Close up and back a little.

I am not to sure you could use a hoe to fix a leaking vent, but, a better description of where the vents are and if you can a picture or two it would certainly help us help you.
