4" Gutter Downspouts

I'm thinking of having my 3" gutter downspouts replaced with 4" sized. Do gutter companies generally increase the gutter hole size, or just use the same hole?

Larger hole if possible. A 3x4 hardly fits well under a 5K gutter. And I'm not 100% sure the outlet will fit there, even after cutting the flasnges off.
