2x6 T&G, insulation, plywood....ughhh...help!

I've never replaced a roof on 2x6 T&G...What's the best way to finish the roof? I've already replaced about 22- 2x6x16 T&G and I'm ready to cover...I'm getting a lot of different opinions...should I cover the decking with 15w felt paper then Insulation. Poly or iso?...what r-value, thickness? What kind of button caps should I use? I plan on going over top of the insulation with 1/2" Ply. Should I screw plywood down or nail it? Should I cover ply with felt? The decking is on beams 4' off center (cathedral cieling) T&G in South Eastern Virginia. Please tell me your opinion on how to do this. 3/12 Pitch. I also plan on using 3 tab 25 year to finish.
