Besides possible spread of mold/mildew & health concerns, is there really any need to recaulk existing caulk that's a bit (but not majorly) moldy? I mean it's not like the caulk will fail & allow water intrusion will it?

Best bet, dig it out and replae it.

If the mildew won't scrub off the surface---but is growing behind the caulk---it is wise to cut out the old caulk and replace--

Mold behind the caulk usually means the bond has failed and is allowing crud to get in there--and grow mold.

Or whatever is behind that caulking is failing. Classic to find drywall used behind tile in a wet area, fails every time.
Any loose tiles, missing grout, do the tiles move when you push on them?
