Water Heater: Water in the combustion chamber - leaking?

This is a 40-gallon natural gas water heater with power vent. Manufactured in 1994, installed in 1996, and well beyond the 10-year warranty.

On Sunday, I flushed the water heater (done annually a preventative maintenance). On Wednesday, the pilot was out. The pilot would light immediately and the burner would re-fire, but a little while later the pilot would go out again. This continued a few times over 2 days.

I'm now realizing that the reason the pilot is going out is due to water or very high moisture levels in the combustion chamber. There is some water visible on a few of the lower surfaces below the burner. Additionally, the lower rim is full of water.

I've confirmed there are no external leaking connections and there is absolutely no moisture problems visible anywhere externally. Is it a good assumption that this thing needs to be replaced, or am I overlooking something?
