Toilet cistern problem


Hello
I wonder some one can help me. Recently I noticed a constant stream of water in the toilet pan coming, I suppose, from the cistern through flush pipe, even when the toilet is not flushed, What is causing this? Is it a flap valve failure in the dome? or the leaky ball valve?. There is no water coming from the over-flow pipe.

What can I do to stop this?

Change out the flapper.
