tile and toilet flange question

I am building a cabin and I am about to install ceramic tile in the bathroom. The floor is 3/4" o.s.b. with hardy backer board over that. From what I have read on this forum the toilet flange will sit on top of the tile after tile is installed. My question is do I just need to cut a whole in the tile large enough for the P.V.C. sewer drain pipe or will it need to be as large as the flange so the top of the flange is level with the tile. How is the flange held in place after it is installed with tile.

The top of the flange can sit level with the new tile or the flange can sit on top of it. There should be holes around the outside of the flange (4 of them) that get screws installed into to hold the flange to the floor. If you do not secure the flange, and the toilet wiggles, it could break the flange or the piping under it.

What he said but I'd add to use brass screws.

If you install the closet flange on top of the tile, you will need to drill holes through the tile to secure the flange.
