Shower Head Breaking Off

On Christmas Eve my mother was taking a shower and the shower head flew off the ball joint causing a severe blunt force eye injury from the impact of the shower head. The ball joint stayed attached to the pipe, just the actual shower head came off. She had noticed the water pressure changing up and down and the temperature changing from cold to hot in an instant since she has lived there, only about 2 months. On the manufactures website is does state that the shower head has a part that reduces the use to 2.0gpm. She lives in a very large apartment complex that is probably about 30 years old. She has been in and out of the hospital with this since injury and surgery and we want to make sure the newly installed shower head is safe before letting her go home. Has anyone seen this before and do you think it is an issue with the water pressure or shower head itself?
Thanks!

An issue with the shower head itself. Cheap parts are never worth it and people never learn. Cheap parts always cost you more in many ways.

Had the ball socket crack on mine last fall. The chrome ball remained on the pipe stem. Plsatic head came off in my hand while adjusting. Plastic socket vs metal ball , plastic will go first. Got a new head under warranty. HANSGROHE, nice folks. Fortunately no injurries.

That is do to the age od the shower head, and its probrably not do to the water saver device inside, which can be removed very easily, if you buy a removable hand held there is no ball bearing deal on there and is very simply instaled with some teflon and smooth pipe wrench. Hope She is all right!
