Kohler bathroom faucet

I have a Kohler Forte two handle bathroom faucet. I installed this faucet about 5 years ago. One of the handle shutoffs is starting to leak where the handle meets the bonnet. I cannot remember how I installed this and I cannot find the installation instructions. This is installed on a granite counter top. If I remember correctly I had to silicone the base of the handles to the granite. How do I get the handles off? I assume that there is probably a stem cartridge of some kind that I will need to replace. Do I need to go underneath the sink and loosen the bolts holding the entire assembly to the counter top or can I remove the handles without removing the whole assembly? Thanks.

Is this a three piece faucet? Typically you won't need to remove the whole thing from the granite. Is there a little allen head screw/bolt holding the handle on? Or is there a sleeve under the handle which screws on the stem? sometimes you unscrew that sleeve and the handle comes off. Good Luck!
