Identify & Fix an Italian ceramic disk sink faucet

I have a leaking Italian sink hot-water faucet (separate taps and spout). Ceramic disk, about 10 mm diameter, with quarter sectors alternately cut out or solid.

Has a "Made in Italy" sticker, but I cannot find any other identifying markings. Faucet design is an old-fashioned, "classic" looking 4-square-handle with rounded ends on the taps, gently curved spout, mounted on the sink itself. Seems to be nickel plated.

Only one of my four taps (dual sinks) is leaking (so far!). Faucets are at least 5 years old, but I'd guess they're more like 10 or 15 years old. Based on the (good) condition of their brass, I suspect that they are not older than that.

Suggestions on where on the faucet to look for the manufacturer, and how to find replacement parts?
