Help ID Laundry Water Valve Cover

Builder left as is. I need a cover but can not find the exact model. Any clues? If I can't find one is it hard to install a brand new complete box? Will some of the dry wall need to be removed to install a new box?

Any suggestions?

I wouldn't replace all of that, which would involve re-plumbing and drywalling.
Just install some off-the-shelf wood trim around the opening with Liquid Nails and tacks, paint it to match the wall, and you're good-to-go.

makes sense. suggestions for the trim? I guess the corners need to be cut on a angle to go together nicely. Do they sell them with the ends already cut?
