Can somone help me identify/fix this housing???

We have bought a new house and need to address a problem with our tub/shower... for reasons not known to us, the former owners took the shower/tub handle and all its components with them. Through a reasonable amount of research, I have not been able to identify what I need in order install a new valve/cartridge/handle/etc. I am attaching a picture so you can see what I am dealing with. The start of the confusion for me is the three ports in the housing. Almost everything I can find only has two ports. Also, I don't know if you can tell in the picture or not, but the two lines on the sides heading downward toward the tub have been cut and capped. The only thing I can find as far as an identifying mark on the rest of the fixtures is a Kohler "K" on the tub faceplate... but I guess that doesn't necessarily mean the handle is the same. All that being said I am feeling at a loss as to what to do next. Do I just need to cut out the old housing, install a new one and start over or can somebody tell me what I am looking at? I would sure appreciate any ideas! Thanks so much for your help!

If you do replace it take the time to get rid of the steel pipe going to the tub.

If you have a part stamped with Kohlers name, call their customer service. You can email a picture to them. They should be able to confirm the model

Any idea why that hole is so big?
Now the whole wall will have to be redone.
If someone was trying to replace it or repair it, it should have been done without having to cut any of that wall.

I am not sure why the big hole... I am wondering if it wasn't one of the three knob type fixtures but I really don't know that. I have found a couple different styles of face-plates that look like they will cover the opening (if I am not mistaken these style face-plates are meant to fill in for a previous three knob fixture). I could be completely off on this though.

A three knob faucet would have had just that three holes not that huge cut out.
Even one of those plates your talking about would not cover a hole that big.
There called renovation plates.

I'm guessing that they were in the process of replacing that mixer---that's why the shower wall have been destroyed.

Some Kohler mixer valves have been discontinued---your may be one of them----I had a customer in that situation---over $300 for the parts----

As long as the wall is demoed---put in a new mixer valve. I suggest Moen or Delta---
