Bathroom basement rough-in

The attached picture is what is roughed in for a bathroom in a basement.

Some obvious items are:
1 - toilet stack.
3 - Main stack (connected to two upstairs bathrooms.

Uncertain items
2 - Appears to be a vent. It travels across the wall horizontally (not shown) past number 3 to some plumbing for a washer and laundry tub.
4 - appears to be a clean-out.

Question:
If I wanted to put in a toilet and sink, where do I connect the sink drain? Can I connect to the vent for drainage? Or do I need to tie into the main stack (3)?

There also appears to be no rough in for a shower if 4 is a clean out? (Larger picture of 4 attached.)

Some of your pipes confuse me--
#1 looks like a clean out plug. Haven't seen toilet stubs look like that
#2 may be a vent. Is it about 42" above the floor and connect to the washer vent? You can probably use it for the lav
#4 looks to me like a backwater valve. Unscrew the lid and see if there is a round flapper in there.

#1 is a toilet stub. I guess they put that on to cap it.
#2 goes from the basement floor (under the concrete) up to the second and third floors. It is a vent for the laundry and appears to also vent the 2 washrooms on the second and third floors.
#4 is a backwater valve.

Question still remains where to tie a sink into? Maybe better off tying into the main stack (#3).

Tie into #2, not the stack. Its looks like the vent for the toilet. It's pretty standard to wet vent a lav there.
