Removing Old Grout

This a strange deal. Shortly after I grouted the tile one of my apt. kitchens, it began to fail. Just in a few areas mind you. Now, some 3 years later, I'd say at least 1/3 of the floor needs to be re-grouted. It must have been a bad batch is all I can come up with. What's the best way to remove the grout? The space between the tiles is about 1/4". Thanks
