Old hardwood plank floors

I tore out carpet in a house that was built in 1890(obviously carpet added later). It had plank floors underneath. I've never done anything like this before so I have several questions. One, how do you fix the cracks in some of the wood planks? 2- do i need to fill in the holes b/w the planks? Is there a way to level the floor w/out tearing up the whole floor? Lastly, there is a big hole in the middle of the floor that they have covered up w/ a piece plywood about a foot in diameter(looks like might have been an old vent pipe). What or can anything be done to fix this? Should I try to use this floor or would it be "easier" to just put in a new floor or new carpet?
